All four lists are computed from verified inspections only — records still pending verification are
excluded so no establishment is ranked on an unconfirmed reading — and cover establishments currently open.
Trend uses a disclosed severity score of 3×Priority + 2×Priority Foundation + 1×Core, averaged per
inspection, comparing the earlier half of an establishment's inspections against the more recent half (minimum four).
An establishment can appear on two lists: a poor overall record that is improving sharply belongs on both.
How to read these records
Priority (P)
Violations most directly linked to foodborne illness — temperature control, contamination, employee illness.
Priority Foundation (Pf)
Missing procedures, records, training or equipment that support a priority item.
Core (C)
General sanitation, maintenance and facility upkeep.
Corrected on site
The inspector recorded the problem as fixed before leaving.
Repeat
Cited on a previous inspection and found again.
Pending verification
Every violation read off a form is checked against the count the inspector wrote in the form's own summary box. These records did not reconcile, or the summary box could not be read, so the reading is unconfirmed and not presented as fact.
No summary box
The inspector left the form's summary box blank, or the scan destroyed it, so there is nothing to check the reading against. The violations are shown as marked on the form, with a note.
